To Qualify for Our Home Health & Rehabilitation Services
The patient must be homebound
The patient may be able to leave home only with great difficulty, infrequently, and for a very short duration.
Candidates for Home Health Care
If homebound,
Having one or more of the following conditions may potentially make the patient a candidate for home health services:

Needs Therapy

Is knowledge deficient of disease process and medication

Has problem with medication administration

Needs Catheter Care

Needs Ostomy Care

Needs Trach Care

Has Pain

Is Urinary/Bowl Incontinent

Needs injection IM/SQ

Needs Blood Draw & Can’t leave home

Has Dyspnea

In addition to one or more of the above, need help with ADLs.
